Participant Forms

Therapeutic riding and carriage driving lessons will continue through October.  Lesson schedules are currently full.  Openings will be filled from our waiting list.  To add a student to the waiting list please download and complete the three forms below.  Please contact us with any questions.  Dates are posted under the lesson sites under "Locations".

"Medical History and Physician's Statement" and "Emergency Contact and Liability Release" forms are required to be completed annually.  A licensed medical provider must sign the "Medical History and Physician's Statement" form.  Please mail completed forms to: River Valley Riders, 8362 Tamarack Village, Suite 119-440, Woodbury, MN 55125.
